How Do I Participate?
By now, you should have received an invitation to participate in this research project. Please complete your registration by logging on to Heartlink.org and following the prompts on the Research Registration box. When your e-mail is confirmed, the survey will be distributed electronically. The information you share will e kept in complete confidentiality.

Important Notes:
If your organization has headquarters with satellite offices, please confirm with your staff as to how you will participate, i.e. headquarters reporting information for each satellite or each satellite completing information individually.

Care Net affiliates: We understand that you prepare an annual report of client statistics for Care Net each year. The good news is that the data you collect for that report will be very helpful to you in completing parts of this research survey.
